Swim Lesson Liability Release & Waiver
By checking the box below, I (the parent or legal guardian) acknowledge and agree to the following:
1. I voluntarily enroll my child in private swim lessons with Zach Hupp ("Coach Zach") and understand that swimming activities involve inherent risks, including but not limited to drowning, injury, or other physical harm.
2. I hereby release, waive, discharge, and covenant not to sue Zach Hupp, his agents, or representatives from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury — including death — that may be sustained during swim lessons.
3. I confirm that my child is in good physical health and has no medical conditions that would prevent safe participation in swim lessons. I agree to inform Coach Zach of any health concerns prior to lessons.
4. I grant permission for Coach Zach to seek emergency medical treatment for my child if necessary, and I accept responsibility for any associated costs.
5. I understand that this waiver is binding upon myself, my heirs, assigns, and legal representatives.
